To help each student with visual impairment to gain maximum independence.

Session Calendar

Returning Short Term students will need the following forms:

  1. Medication Authorization Form - must have physician signature.
  2. Medical History Form - can be completed by parents without physician signature.
  3. Diet Requisition Form - must have physician signature.
All forms need to be mailed or faxed to the Governor Morehead School Short Term Program.
